BIOCHEMICAL PARAMETERS OF ORAL FLUID IN PATIENTS AFTER SKELETAL MAXILLARY EXPANSION WITH OSTEOTOMY

Purpose of the study. To evaluate the effect of a treatment- and-prevention complex, including the drug «Osteovit» and the dental elixir «Lizomucoid», on biochemical parameters of oral fluid in patients after skeletal maxillary expansion with osteotomy. Materials and methods. The study involved 20 patients, who were divided into two groups: a control group, which included 10 somatically healthy individuals, and a main group, which included 10 patients after skeletal maxillary expansion with osteotomy who received the treatment- and-prevention complex. The proposed complex included the administration of «Osteovit» 1 tablet 3 times a day during meals for 2 months, as well as daily use of the dental elixir «Lizomucoid» for rinsing the oral cavity for 30 days. For biochemical analysis, unstimulated oral fluid was used, collected in the morning on an empty stomach. The study was performed dynamically: before treatment, and 1 and 3 months after its initiation. In the oral fluid, the activity of neutrophil elastase was determined as a marker of inflammation; the activity of acid and alkaline phosphatases as indicators of bone metabolism; catalase activity as a marker of antioxidant protection; malondialdehyde content as an indicator of lipid peroxidation; and the antioxidant-prooxidant index. Statistical processing of the obtained results was performed using Microsoft Excel and Statistica 6.0, with calculation of mean values, standard error of the mean, and significance of differences using Student’s t-test. Scientific novelty. For the first time, the positive effect of the treatment-and- prevention complex, including the drug “Osteovit” and the dental elixir “Lizomucoid”, on the normalization of the activity of markers of inflammation and bone metabolism, reduction of oxidative stress intensity, and enhancement of antioxidant protection of the oral cavity in patients after skeletal maxillary expansion with osteotomy has been proven. Conclusions. The conducted studies showed that skeletal maxillary expansion with osteotomy does not lead to pronounced and prolonged disturbances of the biochemical homeostasis of the oral cavity. The use of the proposed treatment-and-prevention complex contributes to a decrease in inflammatory activity, inhibition of lipid peroxidation, enhancement of antioxidant potential, and improvement of bone metabolism indicators. After 3 months of observation, normalization of acid phosphatase activity, an increase in alkaline phosphatase activity, and an increase in the mineralization index were noted, indicating activation of osteogenesis processes. The obtained data provide a biochemical rationale for the effectiveness of using “Osteovit” and the dental elixir “Lizomucoid” in the comprehensive management of patients after skeletal maxillary expansion with osteotomy and allow this approach to be recommended for use in orthodontic and maxillofacial surgical practice.
